Housing Stabilization

Consultation

Developing a housing focused person-centered plan based on the person’s needs, wants, and goals for housing; helping a person make an informed choice in their housing transition or sustaining services provider; offering resources related to non-housing goals; and coordinating with other service providers already working with the person.

Transitioning

Helping the person think about preferred housing; with the housing search and application processes; developing a budget; understanding a lease; and paying for moving expenses.

Sustaining

Education on tenant-landlord rights and responsibilities; coaching to develop relationships with property managers and neighbors; training on how to be a good tenant; lease compliance; and problem-solving to maintain housing stability.

Basic support -coming soon

Individual community living supports (ILCS):

Assistance provided to individuals who require reminders, occasional supervision, or physical aid to enable them to continue living independently in their own homes.

24-hour emergency assistance

Immediate on-call counseling, problem-solving, and response to health or personal emergencies at an individual’s residence.

Companion services

Non-medical care and social interaction provided to adults aged 18 and older to enhance their well-being and companionship.

Homemaker services

Range of eligible services from light household cleaning to more involved assistance with home management and daily activities, based on individual needs.

Night supervision

Overnight assistance and monitoring by awake staff in the individual’s home to ensure safety and support.

Personal support

Services delivered in the person’s home or community to help them achieve personal goals, foster independence, and promote community engagement.

Respite care services

Short-term care provided when family members or primary caregivers require relief or are temporarily absent, ensuring continuity of care for the individual.

With training

This form of Individualized Home Support (IHS) provides “support and training” in community participation, health, safety & wellness, household management, and adaptive skills. The training is aimed at empowering the individual to develop the skills necessary for independent living.

With family training

This form of Individualized Home Support (IHS) offers “support and training” to both the individual with a disability and their family. The training is designed to educate the family on how to effectively support the individual in their home and community.

Employment exploration services

Employment exploration services facilitate understanding of local competitive job opportunities through tailored educational activities, learning experiences, work placements, and relevant support services outlined in the person’s coordinated services and supports plan (CSSP).

Employment development services

Individualized services designed to help a person achieve competitive, integrated employment, become self-employed or establish a microenterprise business in their community.

Employment support services

Individualized services and supports that help people maintain paid employment in community businesses/settings. Employment support services occur in integrated community settings.

Integrated Community Support (ICS)-coming soon

Services providing support and training in community living for adults aged 18 and older residing in provider-controlled ICS settings (e.g., apartments in multi-family housing).

Support includes cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, coordination of community activities, or direct supervision within allowable categories.

Training involves skill-building and instructional services to enhance community living experiences, addressing identified needs.
